On the spine stamped in gold: שער הגלגולים. שער רוח הקודש ושער הגלגולים למהרח”ו - מהדורה ראשונה - דפוס ר' ישראל ב”ק, ירושלים, תרכ”ג - הערות בכת”י בשולי הדפים בספר זה נדפסו השער השביעי - שער רוח הקודש, והשמיני - שער הגלגולים, משמונה שערים שחיבר רבי חיים ויטאל. הביאו לבית הדפוס: רבי יוסף ויטאל והרב יצחק קאלאמארו. מספרי הקבלה המוקדמים שנדפסו בירושלים. בכמה דפים בספר מצויות הגהות בכת”י מתקופת ההדפסה. באחת מהן (דף לד) נכתב: ”ועי' בחו' בב”מ מ”ש בזה”. ע”ח דף. מצב טוב-. נקבי עש בכריכה. נקבי עש (לא רבים) בדפים. על השדרה מוטבע בזהב: שער הגלגולים.
