מחברת דפים בת [18] עמ', המכילה חיבור בעניין עיקרי האמונה. כתיבה אוטוגראפית אשכנזית, ללא ציון שם וחתימה ולא את מקום הכתיבה. [מזרח אירופה, סוף המאה ה-19].
כתב היד נושא את הכותרת "הלכות אמונה" והוא מכיל עשרה סימנים ממוספרים באותיות האלפא ביתא. כל אחד מהסימנים עוסק באחד מעיקרי האמונה עם הרחבה וביאור. סדר העיקרים בכתב היד אינו תואם את 'י"ג העקרים להרמב"ם'. בשולי כתב היד מופיעים נושאים תורניים נוספים.
