דף גדול עם קמיעות, שמות קדושים וציורים קבליים לשמירה והגנה ממזיקים. בצד שמאל למטה "בשנת ברוך אתה לפ"ק - בדפוס ישראל דוב פרומקין וחבירו, מו"ל החבצלת".
