התעודה מעוטרת בתמונות בניין הישיבה ותלמידיה וחתומה (בדפוס) בידי האדמו"ר רבי חיים מאיר הגר בעל ה"אמרי חיים" זצ"ל מוויז'ניץ; לצד זאת היא חתומה בידי ראש הישיבה דאז, האדמו"ר רבי משה יהושע הגר בעל ה"ישועות משה" זצ"ל מוויז'ניץ ורבי יעקב דוד ויז'ניצר ז"ל מנכ"ל מוסדות וויז'ניץ.
