פתק ובו החלטה להושיב את אדון Bronislaw Wohl וארבעה אנשים המוזכרים בשמם הפרטי בלבד (כנראה בני משפחתו) בכתובת מסוימת, אצל אדם אחר. ברוניסלב ווהל מופיע באתרDigitized Collection of Jewish Records כעורך דין בלבוב. בפתק כתוב כי לפי תקנה מתאריך 2 באוקטובר 1941, ההחלטה מחייבת את שני הצדדים - את הדיירים החדשים הנשלחים אל הכתובת וגם את בעל הבית. בגב הפתק מובאים סעיפים 7, 8 ו-11 מהתקנה המוזכרת, ושם שוב אזהרות שההחלטה מחייבת.
גודל: 18.5X14.5 ס"מ.
